PEER SUPPORT TEAM

 

Peer Supports Services are structured and scheduled activities for adults 18 and over with a diagnosis of Mental Health and/or a Substance Use Disorder.

​

Peer Support is an individualized, recovery-focused service that allows individuals the opportunity to learn to manage their own recovery and advocacy for themselves.  Interventions of Peer Support staff serve to enhance the development of natural supports, as well as, coping and self-management skills.  Peer Support Interventions may provide supportive services to assist  individuals in returning to the community following a hospitalization.

Peer Support Services are provided by a Certified Peer Support Staff. Certification involves participation in a 40-hour training that focuses on recovery and wellness management. Peers self-identify as individuals with lived experience and have been diagnosed with a serious mental illness and/or substance use diagnosis. Peer Support specialists help each individual identify his or her needs, strengths and abilities. They support goal achievement and recovery.

PEER SUPPORT SERVICES

​

  • Support growth in the areas of self-help

  • Support growth in the areas of system advocacy

  • Provide pre-crisis and post Crisis support

  • Assist with developing Individualized crisis plan

  • Development of a Psychiatric Advance Directive (PAD).

  • Development of a Wellness Recovery Action Plan (WRAP)

  • Provide support during meetings with providers

  • Assisting the individual with learning how to find and maintain stable housing

  • Support to facilitate growth in the areas of independent living skills.

  • Assisting the individual in gaining information about going back to school or job training

  • Help build natural supports

  • Build peer relationships in a social setting for social skills maintenance and enhancement.

  • Use Recovery philosophies and principles

  • Utilize Motivational Interviewing

  • Utilizes and supports learning in Illness Self-Management

  • Guided by Trauma-Informed Care Model and Principles 

  • Return and add Peers act as: Role Models, etc

  • Role models

  • Supports

  • Liaisons

  • Guides

  • Helpers

  • Motivators

  • Encouragers

  • Listening ears

  • Promoters of recovery

  • Promote trauma-informed care
